Why do some converters differ by a day?

Usually two reasons: incorrect leap year handling, or ignoring time zones. A converter that works in UTC can land a day off for a user who is three and a half hours ahead.

How are Persian leap years determined?

By irregular 33-year cycles defined in a reference table, not the simple divisible-by-four rule of the Gregorian calendar. That is exactly why hand calculation is so error-prone.
